After 1 day of riding. I was in love with the boots. I purchased a size 45, on the advise of the operator at ZLA. I normally wear a 44, however, being a larger American (5'10" 235lbs size 11 shoe), I opted for the 45-wise choice. I was concerned about fitting in the calf. I had to send some Icon boots back because the calves were just too small. The calf adjustment is really a key element to the overall comfort of the Dainese boot. Initially, they seemed a little narrow and slightly tight. Once I got them adjusted and rode them for a day, I was in love. Even in the heat, the gore-tex kept me amazingly dry and comfortable. I was able to walk around for an hour while at a destination, without any problems, then rode for another 4 hours. I was reluctant to spend this much hard earned cash for boots, but they are really worth it- still can't believe I'm saying that, but it's true.
Yes, I recommend this product.

What's the max opening at the top?

I have tree-trunk calves...
So do I (5'10" 235lbs). I had knee surgery that left some permanent swelling in one calf. Having the ability to adjust each boot is great. Both legs can be different diameters- adjust accordingly.

Dainese TRQ-Tour Gore-Tex Boots

I usually wear a shoe size of 9.5 US. I noticed in the sizing chart skips from 9 to 10 what size would be recommended for these boots?
Go bigger. The sizing is not generous. I went up a half size and was happy with the fit.
