I love these boots! The don't have airflow - because they aren't supposed to. I haven't tried them in the rain yet, but they are very comfortable and normal looking as far as protective riding gear goes. The only downer if there is one, is that there aren't any replaceable parts so if you scuff the toe, you're stuck with it. However, that's not the kind of boot I was looking for anyways. Worth every penny!

Dainese TRQ-Tour Gore-Tex Boots
I usually wear a shoe size of 9.5 US. I noticed in the sizing chart skips from 9 to 10 what size would be recommended for these boots?
I wear a 9.5 shoe and bought the size 42. They fit great. Tough call though. I could see a 43 fitting also, but 42 is probably better.

What's the max opening at the top?
I have tree-trunk calves...
Approximately 5.5 inches in diameter at the top (11 inches from the floor). The adjustable part starts at 7 inches from the floor and would probably be about 4 inches in diameter.

Are these "mid shin" boots? What is their height in inches?
10.5 inches from the ground on the heel side and 11.5 inches from the ground on the toe side.
